Minors at your party? Have a parent or guardian sign this form for each child.

How it works

1. Send the form to the parents, in the language they prefer.
2. They fill it in or print it, then sign it.
3. You keep the signed forms.

On a phone, open the file in Files or Acrobat to fill in the fields.

Missions.photos never receives these forms: they stay between you and the parents.
